JOB SUMMARY The Certified Nurse Assistant, reporting to the Nursing Unit Director, with

assignment and direction from the Registered Nurse &/or LVN. This position is directly responsible for

the delivery of high quality individualized basic nursing care. The Nursing Assistant supports the

hospital mission and vision. The Nurse Assistant utilizes knowledge of patient’s age, developmental

status, spiritual and cultural diversity into the provision of patient care. The position contributes to the

provision of safety and quality nursing care that results in positive outcomes in patient care.

  • Sets priorities according to patient care needs.
  • Takes vital signs (temperature, pulse, blood pressure, pain) and reports changes from

established baselines.

  • Recognizes the signs and symptoms of potential emergencies, notifies appropriates nursing

personnel and initiates appropriate actions within scope of practice.

  • Knowledge of care of patient with tubes NG, Gastrostomy, wound draining, O2, IV, chest tube;

Systems(JP & Hemovac)

  • Observes and reports patient's skin for redness/decubitus. Alerts responsible RN for follow-up

care.

  • Observes and reports presence of drainage on dressings to RN.
  • Accurately records patient's intake and output.
  • Participate in aspiration pneumonia prevention; Keep patient upright after feeding. Able to read

the posted signs for thick-it usage.

  • Weighs patients and records accurately.
  • Reports unsafe conditions or problems and offers suggestions for correction.
  • Assists the RN/LVN identifying patient problems on the care plan, in setting new goals and

updating interventions on the nursing care plan.

  • Plans and organizes his/her daily assignments so that all work is completed by the end of shift.
  • To prevent pressure ulcer, turns and positions patients as per posted scheduled, and document

on the chart.

  • Assist patients to wheelchairs or gurneys, and ambulates patients as requested. Demonstrates

practice of proper body mechanics.

  • Administers simple cleansing, oil retention, Fleets enemas, and Harris flushes. Demonstrates

concern of patient comfort and privacy.

  • Delivers trays (include HS snack) to the patient’s bedside; positions the patient upright for meals

and set up for each assigned patient.

  • Provides oral care, bath at A.M., H.S. care and PRN (when patient is soiled). Oral hygiene to

include denture care; bath to include back and peri care.

  • Assists patients in dressing, activity of daily living such as shaving, make-up, and clothing.
  • Assists patient to the bathroom as needed, and assist patients with bedpan or urinal placement

and removal. Patient privacy is observed and patient is left neat and comfortable.

  • Follows hospital Fall Prevention Protocol to ensure patient’s safety.
  • Administers routine catheter care. Empties, measures and documents urine output. Removes

Foley catheters as ordered.

  • Consistently follows universal precautions in handling of patient body fluids. Consistently

demonstrates proper use of hand washing after and before each patient contact.

  • In consultation with the RN, contributes to the plan of care based on interaction with patient

and/or family.

  • Reports to the RN observed patient response to interventions, treatments, medications, and

procedures.

  • Gives accurate, objective, and important information/report to oncoming shift nurse assistant for

the continuing care.

  • Follows Hand-off communication Informs Charge nurse when is off-unit; follows rest (break)

and meal (lunch) schedule informs Charge nurse immediately when able to be on time for meal

period. Informs Charge nurse immediately if a missed rest or/and meal period occurs.

  • Participates in teaching and/or orientating of new personnel.
  • Participates in continuing education in areas that are directly related to the assigned area as

evidenced by written documentation.

  • Operates equipment and performs job related duties in a safe manner.
  • Demonstrates accountability by attending staff meetings, in-services and continuing education

opportunities.

  • Completes annual requirement (self-study module, TB Screen) in a timely manner. Meets

education and competency requirements (BLS, annual competencies, etc.) for position.

  • Assist patients to the restroom and assist patients use bedpan as soon as possible.
  • Quietness of the hospital environment. Minimize talking in hallway, in front of time locks,

elevators. Keep noise down to a minimum when closing doors, using equipment, and during

change of shift.

  • Cleanliness of the hospital environment. Kepp patient’s room, nursing station, and hallway

clean. If trash is visible, pick it up discard appropriately.

  • Performs other duties as assigned.
  • Completion of Nursing Assistant Course, Current certification for Nurse Aide (C.N.A.).
  • High School diploma or equivalent preferred.
  • Six months acute hospital experience preferred.
  • Current Basic Life Support/AHA.
