Primary Care Physician
Provide modern, patient-centered primary care through a combination of in-person and virtual visits. Manage a panel of adult patients with a focus on prevention, chronic disease management, and timely same- or next-day access. Use a technology platform to support efficient documentation, patient communication, and care coordination. Participate in a collaborative, team-based care model with clinical, operations, and technology colleagues.
Key Responsibilities

• Deliver comprehensive primary care to adult patients
• Support prevention and health maintenance
• Manage chronic conditions and ongoing care plans
• Provide timely patient access for urgent needs
• Document care and coordinate follow-up using clinical technology
• Collaborate with cross-functional teams to improve patient experience and outcomes
  

Qualifications & Skills

• MD/DO with appropriate licensure to practice medicine
• Strong clinical judgment in outpatient primary care
• Experience managing chronic disease and preventive care
• Comfort with technology-enabled documentation and care coordination
• Excellent communication and patient-focused approach
• Ability to work effectively in a multidisciplinary team
