Role Description This is a full-time, on-site Family Nurse Practitioner role based in Madera, CA. The Family Nurse Practitioner will provide comprehensive primary care services to individuals and families across the lifespan, including health assessments, diagnosis, treatment, and disease management. Daily responsibilities include conducting patient examinations, ordering and interpreting diagnostic tests, prescribing medications, and developing evidence-based care plans in collaboration with patients and interdisciplinary team members. The role also involves managing urgent care needs, providing patient education on preventive health and chronic disease management, maintaining accurate and timely documentation in the medical record, and coordinating referrals to specialists when needed. The Family Nurse Practitioner will actively contribute to a collaborative, patient-centered environment aligned with Excel Health System’s standards of quality and safety.

Qualifications

• Demonstrated expertise in Family Nurse Practitioner practice, including advanced clinical assessment and decision-making skills (Nurse Practitioners, Nursing).
• Strong foundation in primary care delivery across the lifespan, with experience managing chronic and acute conditions in outpatient settings (Primary Care, Medicine).
• Ability to triage and manage urgent and same-day patient needs in a timely, safe, and efficient manner (Urgent Care).
• Current, unrestricted Nurse Practitioner license in the state of California and national certification as a Family Nurse Practitioner (or eligibility to obtain).
• Graduate degree (MSN or DNP) from an accredited nurse practitioner program.
• Proficiency in electronic health records and documentation standards, with attention to detail and regulatory compliance.
• Effective communication, collaboration, and interpersonal skills, with a commitment to equitable, culturally responsive care.
• Ability to work on-site in Madera, CA, and participate in quality improvement and team-based care initiatives.
