Registered Nurse (RN) Start of Care (SOC)

We are seeking an experienced Home Health Registered Nurse (RN) to complete Start of Care (SOC) admissions in patients' homes. Responsibilities include conducting comprehensive nursing assessments, completing OASIS documentation, developing individualized plans of care, reconciling medications, educating patients and caregivers, coordinating with physicians, and ensuring compliance with all home health regulations and agency policies.

Requirements:

• Active RN license

• Home health and OASIS experience required

• Proficiency with electronic documentation

• Current CPR certification

• Valid driver's license, reliable transportation, and auto insurance
