
Job Summary
We are seeking a compassionate and community-focused Community Health Worker (CHW) to support patients with social, health, and resource-navigation needs. This role serves as a liaison between Cedars-Sinai and the unhoused community, helping individuals access medical care, social services, housing and community resources. The Community Health Worker will conduct assessments, coordinate care, provide ongoing follow-up, and help patients overcome barriers to health stabilization. This position works closely with medical providers, social workers, case managers, patient navigators, community organizations, health plans, and other stakeholders.
Required Qualifications
- High School Diploma or GED
- 1 year of experience in community-level health education or a related field
- Experience providing community outreach, education, navigation, or support services
- Strong communication, relationship-building, and problem-solving skills
- Ability to work independently and collaboratively with multidisciplinary teams
- Ability to work a block weekend schedule, Saturday through Tuesday
Preferred Qualifications
- Bachelor's Degree in Social Work, Psychology, Public Health, Community Health, or a related field
- 2 years of experience working with vulnerable populations
- Experience working with unhoused/homeless individuals or families
- Experience supporting individuals with mental illness and/or substance use disorders
- Experience with community resources, social services, healthcare navigation, or case management
Key Responsibilities
- Build trusting relationships and maintain ongoing communication with unhoused patients, participants, and their support systems.
- Assess social determinants of health (SDOH) and accurately document identified needs.
- Identify social and health-related barriers and develop plans to address moderate disparities.
- Help patients locate, access, and navigate community resources, social services, and medical systems.
- Visit patients in the community or at their homes when appropriate.
- Accompany patients to medical appointments and assist with completing forms and applications.
- Conduct proactive follow-up with patients and families to help reduce barriers to accessing services.
- Collaborate with medical providers, social workers, case managers, patient navigators, and other healthcare professionals.
- Establish relationships with community organizations, social service agencies, faith-based organizations, community centers, government agencies, health plans, and healthcare providers.
- Participate in community engagement activities and outreach programs.
- Identify gaps in available community resources and assist with developing solutions to address those gaps.
- Communicate updates regarding community resources to Cedars-Sinai staff and community stakeholders.
- Work independently on routine requests while collaborating with the internal team on complex issues.
- Provide programmatic and administrative support, including meeting coordination, data collection, data analysis, and other assigned activities.
