Licensed Vocational Nurse (LVN) Home Health \$10K sign on BONUS

About the position

Health Link Home Health and Hospice is seeking a compassionate, organized, and team-oriented Full-Time Home Health LVN to serve patients throughout San Francisco. This role involves collaborating closely with the Director of Nursing, clinical staff, and office team to ensure coordinated, high-quality care. Responsibilities

• Implement the plan of care initiated by the Registered Nurse
• Provide accurate and timely documentation consistent with the plan of care
• Assess and educate patients and caregivers regarding diagnosis, care plans, and treatments
• Participate in coordination of home health services, reporting additional needs (HHA, PT, OT, ST, MSW) to the RN or Clinical Supervisor
• Use equipment and supplies effectively and responsibly
• Complete online charting in Kinnser using company-provided devices
• Follow all company policies and procedures
• Perform other duties as assigned

Requirements

• Minimum 1 year of nursing experience in a medical setting
• IV certification and experience with wound care
• Active California LVN license
• Valid California Driver’s License

Nice-to-haves
