HouseCall MD is a dedicated mobile primary care practice serving geriatric patients in private homes, skilled nursing facilities, assisted living communities, board and care homes, and other residential settings across Southern California.

Position Overview

We are seeking an organized, proactive Registered Nurse (RN) to join our administrative clinical team in our West Los Angeles office.

This is a 100% on-site office-based role focused on clinical care coordination, provider support, and patient advocacy. While our providers care for patients in the field, our office-based RNs serve as the clinical backbone of our operations—managing care workflows, triaging patient needs over the phone, and coordinating with multidisciplinary healthcare partners.

Please note: This is an entirely on-site administrative and care-coordination position; it does not involve remote work or field-based patient visits.

Key Responsibilities

• Care Coordination: Manage day-to-day clinical coordination for geriatric patients receiving mobile primary care services.
• Interdisciplinary Communication: Act as the primary clinical liaison between patients, family members, caregivers, residential facility staff, pharmacies, diagnostic laboratories, home health agencies, and hospice providers.
• Provider & Clinical Support: Assist mobile physicians and nurse practitioners by reviewing provider orders, routing referrals, and addressing pharmacy inquiries or medication refill requests.
• Results & Triage: Review incoming lab results, diagnostic imaging, and clinical updates; triage patient/caregiver calls and escalate urgent medical concerns to treating providers as needed.
• Documentation: Maintain meticulous, timely, and compliant clinical documentation of all communications and coordinated tasks within the Electronic Medical Record (EMR) system.
• Operational Collaboration: Partner with administrative and medical leadership to optimize clinical workflows and improve patient care pathways.

Required Qualifications

• Licensure: Active and unrestricted California Registered Nurse (RN) license.
• Location & Schedule: Ability to work 100% on-site at our medical office in West Los Angeles, CA 90035 (Monday–Friday).
• Clinical Skills: Strong clinical assessment, telephone triage, and critical-thinking abilities.
• Communication & Tech: Excellent verbal/written communication skills and comfort navigating EMR platforms and multi-line phone systems.
• Availability: Ready to start as soon as possible.

Preferred Qualifications

• 1+ years of experience in clinical care coordination, case management, telephone triage, medical office administration, home health coordination, or outpatient clinic nursing.
• Familiarity with geriatric patient care, skilled nursing facilities (SNFs), assisted living communities, or board and care environments.
• Prior experience supporting mobile medicine providers, primary care physicians, or nurse practitioners.

• Pay Rate: $55.00 per hour
