Homecare Licensed Practical Nurse (LPN)

Humancare Home Healthcare Services is a certified home healthcare agency. We are seeking qualified and experienced Licensed Practical Nurses (LPNs) to provide skilled, one-on-one care to adult patients in a homecare setting. These homecare opportunities are ideal for nurses with strong high-acuity experience who are committed to delivering exceptional patient care.

We are currently looking for LPNs to provide homecare nursing services for the following locations > Jamaica Queens and Long Island City Queens for day or nights shifts 12 per day on an on-going basis.

Duties>

• Deliver skilled nursing care to adult patients in a homecare environment
• Manage and monitor high-acuity cases requiring advanced clinical skills
• Perform specialized care including:
• Tracheostomy (Trach) Care
• Ventilator Management
• Gastrostomy Tube (G-Tube) Care
• Nasogastric Tube (NGT) Management
• Administer medications and treatments in accordance with physician orders
• Monitor patient status and report changes promptly
• Maintain accurate and compliant clinical documentation
• Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
