Job Responsibilities: β’ Provide comprehensive primary care, including preventive care, screenings, diagnosis, treatment, and health assessments.
- Conduct physical examinations, obtain medical histories, and develop individualized treatment plans.
- Prescribe and manage medications while monitoring patient outcomes in collaboration with the care team.
- Educate patients on disease prevention, medication adherence, wellness, and healthy lifestyle practices.
- Coordinate referrals and collaborate with physicians, behavioral health providers, and interdisciplinary teams.
Qualifications: β’ Master's degree or Doctorate from an accredited Nurse Practitioner program.
- Current and unrestricted California Nurse Practitioner license.
- Board certification as required for California NP practice.
- Active and unrestricted DEA license.
- Current BLS certification.
- Minimum 2 years of primary care clinical experience required.
- Community health center, FQHC, or underserved healthcare experience preferred.
- Strong clinical, communication, and patient-care skills.
Working Hours: β’ Full-time permanent position.
- Outpatient primary care setting.
- Participation in after-hours on-call rotation.
- Team meetings and clinician cross-coverage responsibilities as needed.
