LVN – Skilled Nursing Facility (SNF)

We are seeking a qualified Licensed Vocational Nurse (LVN) to provide safe, compassionate, and high-quality nursing care to residents in a skilled nursing facility. The LVN will work under the supervision of the Registered Nurse and Director of Nursing.

Position Details

• Position: LVN – SNF
• Contract: 13 weeks, with potential for extension
• Shift: Night Shift
• Pay Rate: $41/hour
• Experience: Minimum 2 years of SNF and acute care experience within the last 3 years
• Location: Lake Isabella, CA 93240 – Kern County

Key Responsibilities

• Assess residents’ conditions and nursing needs and implement appropriate care.
• Administer medications safely according to the 8 medication rights.
• Monitor and manage pain and provide education to residents and families.
• Document resident conditions, care plans, progress notes, and changes in condition.
• Perform and interpret approved point-of-care/waived testing.
• Communicate changes in residents’ conditions to physicians and follow up on orders.

Qualifications

• At least 2 years of SNF and acute care experience within the last 3 years.
• Graduate of an accredited LVN program.
• Current California LVN license.
• Current BLS certification.
• Ability to read and communicate effectively in English.
• Basic computer skills.
• EHR experience preferred.
• Additional language skills are a plus.
