Job Overview

We are seeking a

highly skilled and experienced Nurse Practitioner Family to join our

team. As a Nurse Practitioner, you will play a critical role in

providing high-quality, evidence-based care to our patients. You will

be responsible for managing a diverse range of patient cases,

including wellness exams, acute and chronic illnesses, and preventive

care. This is an excellent opportunity for a driven and compassionate

Responsibilities

  • Conduct

comprehensive health assessments, including physical exams, medical

history review, and diagnostic testing.

  • Diagnose and treat

acute and chronic illnesses, such as infections, injuries, and

diabetes.

  • Prescribe

medication and develop treatment plans in accordance with

evidence-based guidelines.

  • Provide counseling

and education to patients and their families on disease management,

preventive care, and healthy lifestyle choices.

  • Collaborate with

physicians and other healthcare professionals to coordinate care for

patients.

  • Perform minor

medical procedures, such as suturing, wound care, and administering

injections.

  • Document patient

care accurately and thoroughly in electronic medical records.

  • Educate patients

on medication adherence and potential side effects.

  • Monitor patients'

progress and adjust treatment plans as necessary.

  • Participate in

quality improvement initiatives to ensure the delivery of

high-quality care.

Requirements

  • Current and valid

Nurse Practitioner license in the state of CA

  • Master's degree in nursing
  • Minimum of 2 years

of experience as a Nurse Practitioner in a Family Practice setting.

  • Current DEA and

BLS certifications.

  • Excellent

communication and interpersonal skills.

  • Ability to work

independently and collaboratively in a fast-paced environment.

  • Strong critical

thinking and problem-solving skills.

  • Knowledge of

electronic medical records and experience with documentation.

  • Passion for

providing compassionate and patient-centered care.

  • Ability to

maintain confidentiality and professionalism at all times.
