The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) collaborates closely with a Registered Nurse to provide essential nursing care to patients in their homes.

Primary Responsibilities

• Confirm the plan of care with the RN before each patient visit.
• Engage with physicians and RNs regularly to ensure all patient needs are addressed; promptly report any change in patient condition.
• Educate patients and caregivers on effective self-care techniques.
• Monitor and document vital signs.
• Conduct medication reconciliation and review all medications during each visit.
• Perform specific procedures as ordered by the physician, such as foley catheter changes.
• Prepare equipment and materials for treatments following aseptic techniques.
• Ensure documentation is completed accurately and promptly for all visits.

Required Qualifications

• At least 1 year of current LPN or LVN experience.
• A valid LPN license specific to the state(s) you work in.
• Current CPR certification.
• A valid driver’s license, reliable transportation, and liability insurance.

Pay Range

Annual total cash target pay: $59,498 - $89,247

Per visit point rate: $34.33 - $51.49

Hourly rate: $28.60 - $42.91

This total compensation assumes full-time employment (40 hours per week) at full productivity, including earnings from per visit point pay and hourly pay, which are influenced by factors like local labor markets, education, and experience. Compensation varies based on work performed, with pay for certain non-visit activities provided at the hourly rate. Our compliance with minimum wage laws ensures fair remuneration.
