Home Health LVN – Job Summary

Role: Licensed Vocational Nurse (LVN)

Setting: Home Health / Hospice

States: California, Hawaii, Oregon & Washington

Key Responsibilities:

• Provide nursing care to assigned patients in their homes under the supervision of an RN or MD.
• Perform routine home health visits according to agency policies and procedures.
• Perform specialized nursing procedures, including:
• Sterile dressing changes
• Irrigations and cauterizations
• Phlebotomy
• Injections
• Drug screens
• Administer medications as ordered.
• Assist with developing and implementing individualized patient care plans.
• Provide patient education and discharge instructions.
• Coordinate care with the interdisciplinary team.
• Monitor patient needs and report changes to the supervising RN/MD.
• Order and restock supplies according to established levels.
• Provide functional direction to ancillary personnel.
• Maintain cleanliness and organization of assigned areas.
• On-call duties may be required depending on the agency and location.

Requirements

• LVN license in the state of practice – Required
• BLS Healthcare Provider certification – Required
• Arrhythmia certification – Preferred
• High School Diploma/GED or equivalent – Preferred
• Bachelor's degree or equivalent education/experience – Preferred
• Valid driver's license – Preferred
• Must be 21+ years old if required by the agency.
