LVN Field Nurse (Home Health)

Licensed Vocational Nurse (LVN) – Home Health / Field Nurse

Location: West Sacramento & Davis, CA

Pay: $35.00 – $45.00 per hour

Shift: Full-Time

Job Summary

Sonder Healthcare is seeking a Field LVN to provide high-quality home health care to patients across West Sacramento and Davis, CA. The LVN will deliver direct patient care under the supervision of a Registered Nurse (RN), follow individualized plans of care, provide patient and caregiver education, and collaborate with the interdisciplinary team to support positive patient outcomes. This role involves visiting patients in their homes and ensuring continuity of care.

Qualifications

• Graduate of an accredited Practical Nurse or Vocational Nursing program.
• Minimum of 3 years nursing experience; Home health experience required.
• Licensed as a LPN or LVN in California.
• Current BLS certification through American Heart Association
• Strong verbal and written communication and organizational skills.
• Valid driver’s license and reliable, insured transportation for home visits.
• Adheres to professional standards and nursing best practices.

Key Responsibilities

• Implement and follow care plans developed by the RN.
• Maintain accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and caregiver education on self-care, treatments, and disease management.
• Coordinate care with other disciplines (HHA, OT, PT, MSW, ST, Dietician) and communicate patient needs to the RN or Clinical Supervisor.
• Properly use and maintain medical equipment and supplies, adhering to aseptic technique.
• Participate in professional development and maintain competency in home health nursing.
• Perform additional duties as assigned by the RN or Clinical Supervisor.
