Home Health Nurse (Per Diem) Licensed Vocational Nurse - LVN

Provide skilled nursing care services to patient in their homes for a duration of 4 times a week for 8 weeks, following physician orders and the individualized plan of care. Responsibilities include head to toe assessment and medication management for the patient and caregiver education, documentation, and communication with the interdisciplinary care team to promote optimal care.

Requirements:

• Active LVN license (as applicable)

• Home health experience preferred

• Current CPR certification

• Reliable transportation and valid driver's license

• Excellent documentation and communication skills
