Home Health Wound Care Nurse (Per Diem) - Licensed Vocational Nurse - LVN

Provide skilled wound care services to 1 patient in their home TWICE A WEEK FOR 7 WEEKS, following physician orders and the individualized plan of care. Responsibilities include wound assessment, dressing changes, infection prevention, patient and caregiver education, documentation, and communication with the interdisciplinary care team to promote optimal healing.

Requirements:

• Active LVN license (as applicable)

• Home health experience preferred

• Wound care experience required

• Current CPR certification

• Reliable transportation and valid driver's license

• Excellent documentation and communication skills

• Visit episode (TWICE A WEEK FOR 7 WEEKS)
