LVN Home Health Field Nurse

Our Agency is looking for a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our team in Palmdale, CA and surrounding areas. Our Agency services adult and geriatric clients on a per-visit basis in territories throughout Los Angeles County, Orange County, and Kern County.

One year of prior clinical experience as a licensed LVN in Home Health is required.

Licensed Vocational Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors and teaches and educate patients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RNs, Administrator, and Director of Patient Care Services

Duties

• Administer medications and treatments as prescribed by physicians
• Monitor and record vital signs, symptoms, and changes in patients' conditions
• Assist with diagnostic tests and procedures
• Collaborate with healthcare professionals to develop and implement patient care plans
• Educate patients and their families on healthcare topics, including disease management and preventive care
• Maintain accurate and up-to-date medical records
• Adhere to infection control standards to ensure a safe and clean environment for patients
• Provide acute pain management and suture care as needed
• Assist with pain management techniques
• Experience working with children is preferred

Requirements

• Valid LVN license in the state of California
• Basic Life Support (BLS) certification
• Knowledge of airway management techniques
• Familiarity with Durable Medical Equipment (DME)
• Strong communication and interpersonal skills
• Attention to detail and excellent organizational skills
• Wound Care

Job Types: Part-time, Temporary, Contract

Pay: $55.00 per hour
