Full-Time Home Health LVN (Dedicated Route) – Company Vehicle & Driver Provided

Compensation: $5,700 per month (1099 Independent Contractor)

Schedule: Monday–Friday | 8:00 AM–6:00 PM

Location: Southern California (Assigned Service Area)

Velgato is seeking a dependable, compassionate, and highly organized Licensed Vocational Nurse (LVN) to join our growing home health team. This is a unique opportunity to focus on patient care while we handle the driving and logistics.

Responsibilities

• Perform skilled home health visits in accordance with physician orders.

• Complete accurate and timely electronic documentation.

• Assess patients and report significant changes to the supervising RN and physician.

• Provide patient and caregiver education.

• Communicate professionally with physicians, case managers, and office staff.

• Maintain compliance with all applicable home health regulations and company policies.

Qualifications

• Active California LVN license in good standing.

• Current CPR/BLS certification.

• Minimum of one year of clinical nursing experience (home health experience preferred).

• Strong assessment, documentation, and communication skills.

• Professional, reliable, and patient-focused.

• Ability to efficiently complete scheduled visits throughout the workday.

Sign-On & Retention Bonus

Earn up to $7,500 in bonuses:

• $2,500 Sign-On Bonus – Paid after successfully completing 90 days of continuous service.

• $2,500 Retention Bonus – Paid after 6 months of continuous service.

• $2,500 Loyalty Bonus – Paid upon successful completion of 12 months of continuous service.

Bonus Eligibility & Repayment Terms:

• Maintain active contractor status and remain in good standing throughout each eligibility period.

• Meet all performance, documentation, attendance, and compliance standards.

• Bonuses are paid only after each milestone is successfully completed and are never paid in advance.

• If the Independent Contractor voluntarily terminates the Agreement or otherwise fails to complete the twelve (12) month commitment, all previously paid sign-on and retention bonuses shall become immediately due and payable to Velgato.

• The Independent Contractor expressly authorizes Velgato, to the extent permitted by applicable law and the Independent Contractor Agreement, to offset any bonus repayment against any amounts otherwise owed to the Independent Contractor. Any remaining unpaid balance shall be repaid to Velgato within thirty (30) days of termination.

• Velgato reserves all legal and contractual remedies to recover unpaid bonus amounts.

• Additional terms and conditions are set forth in the Independent Contractor Agreement.
