Position Overview

Velgato Health is seeking a reliable and experienced LVN to provide dedicated one-on-one skilled nursing care for a pediatric/adult patient requiring specialized respiratory and enteral support. This is a consistent overnight case focused on maintaining patient safety, comfort, and quality of care in the home setting.

The ideal candidate has hands-on experience managing tracheostomy care and G-tube feeding and is confident working independently during overnight shifts.

LVN Responsibilities

• Provide skilled nursing care for one assigned patient in a home care setting

• Perform tracheostomy care and monitor respiratory status

• Manage G-tube feedings, medication administration, and enteral care

• Monitor vital signs and identify changes in patient condition

• Follow physician orders and individualized care plans

• Document all skilled nursing interventions accurately

• Maintain infection control and safety standards

• Communicate patient updates and concerns to the care team

• Provide compassionate, patient-centered care throughout the shift

Required Qualifications

• Current California LVN license (Active and in good standing)

• Current CPR/BLS certification

• Previous experience with:

  • Tracheostomy care

  • G-tube management

  • Home health or private duty nursing

  • Medically complex patients

• Strong clinical judgment and ability to work independently

• Reliable attendance and commitment to scheduled shifts

Hours: 10:00 PM – 6:00 AM
