VitalEdge Staffing is seeking a licensed Occupational Therapist to join a respected PACE (Program of All-Inclusive Care for the Elderly) organization serving seniors throughout the San Diego area.

In this role, you'll work as part of an interdisciplinary care team dedicated to helping older adults remain safely and independently in their homes and communities. This is an excellent opportunity for an OT who enjoys working with geriatric populations and wants to build lasting relationships with participants while providing individualized, holistic care.

What You'll Do

• Conduct comprehensive occupational therapy evaluations and assessments.
• Develop individualized treatment plans to maximize independence and quality of life.
• Provide therapeutic interventions to improve functional abilities and daily living skills.
• Recommend adaptive equipment and home modifications.
• Educate participants, family members, and caregivers on safe and effective strategies.
• Conduct home visits and community-based assessments as needed.
• Collaborate closely with physicians, nurses, social workers, therapists, and other interdisciplinary team members.
• Monitor participant progress and adjust treatment plans accordingly.
• Complete required documentation and maintain accurate clinical records.
  

QualificationsRequired

• Current California Occupational Therapist License
• Graduate of an accredited Occupational Therapy program
• Minimum one year of Occupational Therapy experience
• Experience working with geriatric or senior populations
• Valid California driver's license and reliable transportation

• PACE experience
• Home Health experience
• Bilingual English/Spanish
• PAM Certification
