Key Responsibilities

• Coordinate with physicians, rehabilitation services, and other healthcare providers.
• Educate clients on their diagnoses and treatment plans for better understanding.
• Provide skilled nursing services, including wound care, medication management, and chronic condition management.
• Maintain organized documentation of care provided.

Requirements

• Current, unrestricted California LVN license.
• Graduation from an accredited practical or vocational nursing program.
• Minimum of 1 year of experience as a Licensed Vocational Nurse.
• Home health experience preferred.
• Proof of negative TB result.
• Physical screening documentation within the last year.
• Current BLS certification.
• Valid California driver’s license and insurance.
• Cell phone with internet access.
