Title: LVN - Hospice / Skilled Nursing

Start: ASAP

Duration: 13 weeks

Shift: Days, 8:00 AM-4:30 PM

Schedule: 5×8-hour shifts; must work weekends + 3 weekdays

Job Description

Seeking an experienced LVN to provide hospice and skilled nursing care to patients under RN/MD supervision. Responsibilities include routine patient visits, medication administration, clinical procedures, wound care, patient/family education, care-plan implementation, and coordination with the interdisciplinary care team. On-call duties may be assigned based on agency needs.

Qualifications

• Active LVN license in the state of practice
• BLS certification required
• Recent hospice experience required
• Epic experience required
• Experience with medication administration and clinical procedures
• Strong patient-care and communication skills
• Must be available to work weekends and 3 weekdays
• Valid driver’s license required
• Work Location: In person
