About HouseCall MD

HouseCall MD is a mobile primary care practice serving geriatric patients in private homes, skilled nursing facilities, assisted living facilities, board and care homes, and other residential settings.

Position Overview

We are seeking a Registered Nurse to support our mobile providers and coordinate patient care.

This position is

• 70% to 80% office-based in West Los Angeles, CA 90035
• 20% to 30% field-based throughout the San Gabriel Valley

Patients are not seen in our office. The office-based portion focuses on virtual patient care coordination and clinical support. The field-based portion involves traveling to patient homes and facilities to provide direct nursing services.

This is a new position that will continue to develop based on the needs of our patients and providers. Candidates must be comfortable working in both settings.

Responsibilities

Office-Based

• Coordinate care for geriatric patients receiving mobile primary care services
• Communicate with patients, families, caregivers, facilities, providers, pharmacies, laboratories, home health agencies, and other healthcare partners
• Review patient updates and escalate clinical concerns appropriately
• Follow up on provider orders, laboratory results, medications, referrals, and other clinical needs
• Support physicians and nurse practitioners with patient care coordination
• Document communications and completed tasks in the electronic medical record

Field-Based

• Travel to patient homes and facilities throughout the San Gabriel Valley
• Perform nursing assessments and report changes in condition
• Provide ordered nursing services, which may include wound care, IV therapy, vital signs, medication review, and patient education
• Coordinate with providers, caregivers, families, and facility staff
• Complete accurate and timely documentation after each visit

Required Qualifications

• Active and unrestricted California Registered Nurse license
• Willingness to work in both the West Los Angeles office and the San Gabriel Valley field territory
• Valid driver’s license and reliable transportation
• Strong clinical judgment, communication, and organizational skills
• Experience providing direct patient care
• Comfortable working with geriatric patients and healthcare technology
• Available to start as soon as possible

Preferred Qualifications

• Experience in care coordination, case management, telephone triage, home health, mobile medicine, or geriatric care
• Experience with skilled nursing facilities, assisted living facilities, or board and care homes
• Wound care or IV therapy experience
• Experience supporting physicians or nurse practitioners

Compensation and Work Location

• $55.00 per hour
• Full-time position

• Field territory: San Gabriel Valley and nearby areas
• Approximately 70% to 80% office-based and 20% to 30% field-based
• This is not a remote, office-only, or field-only position
