What You’ll Do

• Provide skilled nursing visits (Wound care, IVs, injections).
• Report status changes to RN Case Managers in real-time.
• Educate families and patients on disease management.
• Submit clear, timely electronic documentation.

Qualifications

• Active California LVN License (Required).
• 1+ Year Clinical Experience (Home Health experience is a plus, but we will train motivated LVNs).
• Road Ready: Valid DL, reliable vehicle, and auto insurance.
