RN for Home health (Experience with Veterans preffered)

• We are seeking a Registered nurse for Home visits. You will be responsible for the assessment, diagnosis, and treatment of assigned patients in a home health setup.

Please note that our rates are per visit-not per hour.

Responsibilities

• Providing nurse visits to home health patients. (home visits)
• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Demonstrates an understanding of military and Veteran culture and how military experiences may influence healthcare-seeking behaviors, communication, and treatment adherence. Provides culturally informed, respectful care by recognizing the unique challenges, strengths, and healthcare needs of Veterans.

Home visits for EVAL/ASSESSMENT

• Documentation (oasis)
• IV therapy (when needed)
• Wound care
• Patience, communication skills
• English is mandatory ( ANY OTHER LANGUAGES ARE HIGHLY PREFERRED.

Experience in HOME HEALTH CARE highly preferred

• $120 for start of care. (may be higher depending on location)
• follow up $ will be discussed
