• Local: $68.89/hour | Approx. $2,756/week

Job Summary

Seeking an experienced RN Case Manager to manage a geriatric patient caseload, coordinate interdisciplinary care, develop care plans, provide direct nursing care, and support effective utilization of healthcare services.

Requirements

• Active California RN License
• Current BLS
• 1+ year of RN experience
• Home Health Case Manager experience required
• Previous charge experience required
• Epic and care coordination experience preferred

Key Responsibilities

• Manage assigned patient caseload and care plans
• Coordinate interdisciplinary patient care
• Provide direct nursing care
• Support discharge planning and disease management
• Maintain accurate clinical documentation
