Intake Coordinator Registered Nurse

Coordinate the intake process for patients seeking treatment at our facility.

Responsible for accurately and efficiently gathering patient information and verifying insurance coverage.

Conduct initial assessments of patients to determine appropriate level of care.

Collaborate with the treatment team to develop individualized treatment plans for each patient.

Provide excellent customer service to patients and their families throughout the intake process.

Maintain thorough and organized records of all patient interactions and admissions.

Communicate effectively with patients, families, and outside referral sources to ensure a seamless admission process.

Utilize knowledge of insurance guidelines and regulations to ensure proper documentation and authorization for admission.

Serve as a liaison between patients, families, and the treatment team to ensure a smooth transition into the facility.

Participate in interdisciplinary team meetings to discuss patient care and progress.

Serve as a resource for patients and families, providing information about treatment options and answering any questions or concerns.

Identify and report any potential barriers to patient admission or treatment.

Adhere to all legal and ethical standards in regards to patient confidentiality and privacy.

Continuously seek out opportunities for professional development and stay up-to-date on industry trends and best practices.

Promote a positive and collaborative work environment within the intake team and with other departments.
